     The mission of HearthStone is to be a self-help organization providing information, guidance, and support to all individuals with developmental disabilities, their parents, guardians, and advocates in support of community opportunities. The organizational focus is in the following areas:

ASSESSMENT / REFERRAL SERVICES – HearthStone will facilitate “person-focused” planning services.  The plan will strive to meet individual support, residential placement, vocational, and recreational needs in an integrated community setting.  HearthStone will provide information and referral services to connect the needs of clients and family with community resources, support providers, and organizations.

RESIDENTIAL FACILITIES – HearthStone will assist in the identification and acquisition of community residential settings which are appropriate to the disabled individual’s needs.

PERSONAL DEVELOPMENT – Learning opportunities in vocational, social, and recreational skill development shall be offered to HearthStone members.

RESPITE – HearthStone, in collaboration with other Sheboygan County community resources, will work to provide a respite cost-reimbursement program for families with developmentally-disabled members.
